Update Readme to match more distributive bot

The old readme was good as it was for a bot I was using personally, but I plan to modify this bot to allow anyone to host it for their own guilds, therein I will need a README to match.

Made swapping the monitoring mode, and messages more robust so that they can be swapped to meet any use-case the bot may be used for.
